Create an Intervention
1. Click Insert a Row in the Intervention section .
2. If you are member of more than one team, the Owner key look window appears. Select the team associated with the Need & Goal. If you are only a member of one team, then the owning team is defaulted into the Needs & Goals entry form.
The 'Interventions - entry' window appears:
3. Enter the following fields:
- Intervention: Select a Intervention from the lookup
- Details: This is an optional unlimited free text field used to enter details about the Intervention.
- Discipline: Is autopopulated with the professional discipline of the logged in user, but can be changed
- Date Identified: Is defaulted to the date the entry is being created
Link Intervention to a Need & Goal
4. Click the button. The link goals window appears:
NOTE: The Link intervention to goal(s) button must be used to link the new Intervention to one or more existing Goals or else an error message will appear when you click Accept Changes.
5. Select one or more Goals from the Link to goals window and click Select.
6. The selected goals display in a list under the Linked [Need] Goals heading, in the format: [NEED] GOAL.
7. Click Accept Changes. The new Intervention record will now exist in the Interventions grid section and will have a status of ‘ACTIVE’, indicating it does not yet have an end date recorded.
8. Click Save. The linked Needs & Goals display in the Active Care Plan Summary section near the top of the form.
NOTE: After saving the new record all fields are read only except for: Details, End Date, and Discipline.
Modify Care Plan Interventions
The details of an Intervention may be changed at any time providing the Intervention does not differ enough to warrant a different entry with another pick-list value. The Intervention details should contain only current information. The previous documentation for a particular intervention may be viewed by selecting Version History.
1. Go to the Clinical Care Plan application
2. Go to the Intervention grid, highlight the Intervention you wish to make changes to and then click modify row.
3. In the Details section, add any information that is required. Delete any information that is no longer relevant.

NOTE: It is a known issue in the system that sometimes when the Intervention Details in the CCP are modified, the Last updated by name and date in the Interventions-entry window do not automatically update upon saving.
If the Intervention Details are the only thing that’s modified in the CCP, then the Care plan last updated name and date will also not update upon saving. This information is updated by the following day.
If a change to the intervention details needs to be communicated the same day, please consider an alternate method of communicating the change (e.g. PARIS notification, email, phone call).
Version History will always update upon saving and immediately reflect the most current updated date, time and name.
