Pass Referral to Another Team
New Referral
If you're logging a new referral, start by:
- Search for a client using the
function.
- If you do not have access to the client's record you will need to Break Glass.
- Locate the Referrals application either through the Referrals or Shortcuts tiles.
- In the Referrals Application, select
in the Referral toolbar. Use the Owner of new referral lookup
.
- If you belong to multiple teams, the Owner Key Lookup window appears. Select the team you wish to make the Referral to.
- If you're logging a referral to another team you don't belong to: Select YOUR team. The referral can be passed to another team by following the Pass Ownership instructions after completing the following steps.
5. Complete the mandatory fields in the Referral Details section.

- If you are logging a referral an external team and are unfamiliar with the process of unsure of the details for the new referral (appropriate team, priority, etc.) you should discuss with a member of the receiving team before completing the referral.

- Referral Type: Defaults to a Referral Type for your team. If referring to another program, update this field.
- Referral Date: Press Ctrl + T for today’s date or F4 to launch calendar if entering prior date.
- Referred to: Defaults to the currently selected team name. To refer to another team, enter the team name.
- Priority: Select the Priority as per your program guidelines (e.g. TST- VGH, Not Applicable, etc). Type and double-click to select from picklist or type the code and press tab. If you are unsure of the appropriate priority consider discussing with a member of the team you wish to make the referral to.

Referral Reason: This can be entered in two different ways:

- Use the lookup tool by clicking the
(or using F4) and type the reason in the Type & Find, then select from the list (e.g. Entering “Consult” lists all referral reasons containing that term).
- If known, start typing the referral reason code then select from the picklist.

Source: Select the appropriate Referral source as per your program guidelines. Click the
(or use F4), then use Type & Find or scroll through the picklist and double-click to select the appropriate source (e.g. self, physician, internal team). Alternatively, the code (if known) can be entered in the Source field (e.g. THCC for INTERNAL TEAM – HOME & COMM CARE).

- Internal Team is the most common referral source, and allows you to enter your team and name.

- Click Select.
- Depending on the source selected a confirm box may appear asking if you wish to search for details. Follow your local guidelines on entering these details.
- Once you select the source, the information will be pulled into the Referring Source Details section.
6. Complete other sections of the referral as appropriate
- Tick the Client and Family aware boxes as per your program guidelines
7. Click or press F12. After the referral is saved, the Options Tree displays the referral under your own team with the Referral Reason you have just selected.
After logging a new referral the clinician should enter a referral casenote.
8. To enter a referral casenote, click on the button in the Referral toolbar. Your casenote should include any pertinent information. If the referral was discussed with another member of the interdisciplinary team (e.g. Triage, CRN, etc.), your casenote should also include a summary of the conversation as well as their name/role.
9. If you are referring to a Paris team you do not belong to you should follow the Pass Ownership instructions.
Discharge Planning referral details for Vancouver/ Richmond Home Health
Reminder: If you’re logging a Referral to pass to a PARIS team you don’t belong to (i.e. you’re making a Referral to another health unit or to TST), you are required to first select a PARIS team you do belong to within the Owner Key Lookup window. The Referred To field will then need to be changed as below.
Referred to:
- For Vancouver: Transition Services Team
- For Richmond: CC-RMD Community Transitions
Only 1 Discharge Planning referral is required per client per hospital visit (e.g. If client is open to NSG, CM and PT only one Discharge Planning referral is required)
Referral Reason: Discharge Planning
Source: “Make me the source” and then choose Internal team – Home and Comm care
Priority:
- For Vancouver: TST and appropriate hospital initials (e.g. TST – SPH for Transition services at St Pauls Hospital).
- For Richmond: Liaison – Richmond
- Add a casenote detailing relevant collateral information (review appendix A in the Community to Acute Communication SOP on oneVCH.ca for details to be included).
Notify:
- Notify other involved staff
- If staff is already aware, a notification is not necessary (e.g. discussed in person)
- Notify button is available in PARIS to notify involved staff (e.g. key/co providers)
To complete the Discharge Planning Referral process, you must Pass Ownership of the Referral in the Actions Taken grid. See the Pass Ownership article.
Pass Referral to Another Team
Reminder: If you’re logging a Referral to pass to a PARIS team you don’t belong to (i.e. you’re making a Referral to another health unit or to TST), you are required to first select a PARIS team you do belong to within the Owner Key Lookup window. The Referred To field will be changed in a later step.
- Follow the steps above for entering a New Referral. You should write your referral casenote before passing the referral.
