When a service is no longer needed, any open Assessments, Clinical Care Plan Needs/Goals/Intervention or PAGs/DOTs need to be closed prior to discharging the Referral
- You can’t discharge a Referral if there are open attached Assessments– you will get an error message, “Unable to Discharge Referral – Open Assessment linked to referral”
- If you see “Open Care Plan Interventions linked to Referral”, this means there is an active Home Support Service Order. Please ensure any HSO associated with the client’s Referral is appropriately end-dated. If all HSOs appear to be end-dated and ‘Inactive’ but you’re still unable to discharge your Referral, please contact the PARIS Help Desk
- Discharged Referrals display in green text in Referral Options Tree and the status changes to Closed
- Discharging a Referral will end date ALL Involved Staff and remove the client from everyone’s caseload (Does not apply to Long-Term Care).
Special Note on Funded Services
For all funded services - do not discharge the referral that the service is attached to until notified from PA that all interventions have been documented in the Financial Care Plan.
In some instances, clinicians will discharge Funded Services Referrals. Check with your local leadership prior to discharging.
(This could be several weeks after the service ends for some services such as ADP, as the PA has to enter information about the client attendance)

Discharge an Active Client's Referral
1. Click on the Discharge Application.
2. In the Discharge Options tree, click on the date under the Referral you need to discharge.

3. If you get the message “Referral Cannot be Discharged”, it could be for one of two reasons. The reason would be given in the information section.
- If it says: “Open Assessment linked to referral”, close all assessments attached to the referral. See how to Close an Open Assessment.
- If it says: “Open Care Plan Interventions linked to referral”, for Home Support intervention types end intervention via the Home Support Order form. For all other intervention types, end date directly in the Care Plan.
4. Once all Assessment and Care Plan Needs are closed, the Open Assessment and Open Needs (from Care Plan tabs will default closed. For Home Health clinicians, open-ended HSOs need to be end-dated.
The Discharge Details section will now appear:
5. To discharge the referral, enter the following fields:
- Discharge On: Defaults to today’s date, if this is incorrect, enter the correct date of discharge

Destination: If destination is required by your program, select the appropriate Destination from the picklist by clicking the
6. Press F12 to Save. The discharged referral is now displayed in red as it is no longer available to be discharged.
Discharging a Client with an Active task linked to referral
Tasks are created activities that are linked to referrals. They are not used in Home Health and home health clinicians should be not be creating them. Sometimes a home health clinician accidentally creates task and when this happens the referral that the task is linked to can not be discharged until that task is closed.
1. Confirm the key provider allocated to active referral.
2. Navigate to the Team Caseload tile.
3. Expand Active options tree. Locate the PARIS team the active referral belongs to and then expand that options tree as well. Locate the name of the key provider and then expand their options tree. Locate the client under the key provider’s options tree.
4. Click the client’s name.
5. On the Details screen scroll down to the Tasks grid and expand that grid.
6. Select the open task and then click Modify row.
Only the staff member listed under the Staff field can end date the task. For all other users the End date field will be locked. If the End date field is locked change the name in the staff field to your name.
8. Enter an end date in the End date field.
9. Repeat steps 6-8 for all other open tasks linked to the referral.
10. Go back to the Discharge tile and discharge the referral.
Discharge a Deceased Client
If the client has been marked Deceased, a Reason of “Client has Died” defaults into the Reason field. This Reason is Read Only and can’t be edited
The 'Discharge On' date defaults to the date the client was marked deceased. This date can be changed.
You must make a change to enable the Save function!
1. To discharge the deceased client, enter the following field:

Destination: Click
or press F4 and select Other from the picklist (You may either leave Other in Destination or delete it).
2. The Save function is now available, press F12 to discharge your Referral.
Wrong Discharge Date, Reason or Destination
1. Click on the Discharge application
2. Referral will NOT be visible. Click on the button in the Discharge toolbar
3. Find and click on the Red date text in the Options tree which you need to edit
4. Update the following fields:
- Discharge On: Enter the correct date
- Reason or Destination: Select the correct entry from the picklist
5. Press F12 to Save
