For universal screening, or when someone other than the clinician conducting a particular assessment has created the assessment, the clinician must remember to change the 'Carried out by' field in the given assessment details tab.
1. Open and activate the appropriate client's record and go to assessments.
2. Select the appropriate assessment.
3. Click on the Details Tab on the bottom left of the assessment.
4. Change the 'Carried out by' field to your own PARIS username if you are the clinician who is completing the assessment. You may do this by removing the user's name who is currently in the 'Carried out by' field and adding in your own username.
5. Accept Changes and Save. You may now complete the particular assessment under the other tabs.
