Maternal Newborn Client Search
To Search for a client click on
at the top of the screen. Always use the PARIS Search Procedure's 3 steps:
NOTE: Remember to clear any information in the search field before you enter your search
1. PARIS ID
2. PHN
3. Last name*, First initial of Last Name *
For babies, check using mom and dad's last names - Usually under mom's.
See Register Newborn if infant cannot be found. See Register New Client to register mother.
Client Referral
1. Check to see if there is an open referral for the client as the client may already have one.
2. To create a new referral click on button
in the Referral toolbar.
- The Owner Key Lookup window appears
- Highlight the correct team who will own the referral, press enter or click Accept Changes
- For discharged clients, use the correct team (Usually the commonly used team name i.e. PH-North Shore)
- For clients that remain in hospital that need to be pre-registered (i.e. babies that remain in the NICU) use the liaison team (i.e. PH-NS Liaison)
3. To create the referral, enter all the mandatory fields:
- Referral Date: Should be the day you received the data

Referral Reason: Select the appropriate referral reason from the picklist:
- For Mom: PHN-Maternal; PHN-Maternal Midwifery are most commonly used
- For Baby: PHN-Newborn; PHN-Newborn Midwifery are most commonly used

to activate the picklist and search for the birth hospital. If a home birth choose "Other" - Priority: If the hospital is within VCH use the picklist and select "PH-liaison- [Hosiptail i.e. Lions Gate, Womens or SPH) Home births and referrals from hospitals not listed select "NOT APPLICABLE"
Open an Assessment
1. For new maternal assessment code: enter “ICPM” – “Postpartum/Maternal”
2. For newborns/infants – assessment code is “ICPN” – “Postnatal/Newborn”
- Enter Birth weight, length and head circumference in the growth chart
NOTE: Follow site specific workflows related to data entry into the PP/Maternal Liaison Referral and Postnatal/NB Liaison Referral
Client Discharged
1. When clients are discharged from hospital, go to existing referral and change the “Referred To” from Liaison team (select your appropriate team, i.e: PH-NS Liaison) to commonly used team name (select your appropriate team, i.e: PH-NS North Shore) and scroll down screen to Actions taken. Open a row and select Pass Ownership and then SAVE.
2. Do not pass ownership on clients who remain in hospital (i.e babies who remain in NICU while mom is discharged)
3. Remember to allocate the referral according to your site specific workflow
