Add a New Contact
Contacts can be added in the General CD Edit tile when a new case is created or an existing case is selected via the General CD Search tile. For help searching for a specific case, please refer to the Search for a Case or Contact page for more details.
1. Click on Insert a row in the Contacts grid.
2. Enter all details in the Contacts window, if known. Depending on the communicable disease, there are different fields that may or may not appear in the form and some of the pick lists are filtered based on the selected disease.
If a letter was sent, in the PH Intervention(s) section check Letter Sent. Attach File button becomes visible.
Click Attach File. User’s home directory (H:) displays. For instructions on how to save a file to your Home Drive, please refer to the Save a Scanned Assessment File to your Home Drive demonstration video.
Select letter (pdf file). To verify you are attaching the correct file, right mouse click on the file. Select “Open”. Once verified, close the file. Click Open to attach the file.
Any comments related to the investigation can be entered in the Additional Comments section at the bottom of the form.
Some important fields include:
| PARIS ID |
Click on the magnifying glass to search for the contact in PARIS. If the contact is in PARIS, their PARIS ID, last name, first name, birth date, gender, PHN, primary phone, email, and address will populate from Central Index. This is not mandatory, but can save time entering client information. |
| Proxy Details | Enter the designated contact person and their contact details here, if applicable. |
| Staff Safety Concern | Check the box if there is a safety concern and type in the free-text box where more details can be found (e.g., PARIS Community Risk Screen, Additional Comments, etc.). |
| Successful Method Of Contact | Enter the method of contact that was used to successfully reach the client. |
| Part of Group Contact | Check the box if the contact is identified from an existing group contact. |
| Name of Grouping | If the contact is part of a group contact, select the group or type in the name of the group contact here. |
| Medical History | Record the contact’s medical history as it pertains to the risk of disease or transmission. |
| High Risk for (Severe) Disease | Indicate here whether or not the contact is at high risk for disease or severe disease, as defined by the disease-appropriate BCCDC guideline. |
| High Risk for Transmission | Select Yes if the contact has other contacts who are high risk or if the contact is at high risk for transmitting for a reason unrelated to occupation or attendance at childcare/school (e.g., exposed to Hep A and has high risk sexual practices; exposed to measles and lives with an infant). |
| High Risk Setting | Select Yes if the contact has an occupation or attends a setting where there is a high risk for transmission (e.g., exposed to Hep A and is a food handler; works as a nurse in the NICU). Select the appropriate setting in the If yes, Details question. |
Please note that the Investigation Outcome and PH Interventions sections should only be completed when contact investigation is complete.
3. Click on Accept Changes and Save to save the contact details.
To view the attached letter, the file can be viewed one of two ways:
- Highlight Contact. Click View File button. File displays.
- Highlight Contact. Click View Details or Modify Row. Click View File button.
Edit a Contact
1. Select the appropriate contact from the Contacts grid and click on Modify row.
2. The Contacts window opens and contact details can be added or modified.
3. If the wrong letter was attached to the contact, check Entered in Error checkbox.
4. Click on Accept Changes and Save to save your changes. Note, the filename will display in the erroneous record but the file is permanently removed.
5. Click More Actions and select Show Errors. Click Copy current row. Letter Sent checkbox and file do not copy.
6. To replace file, check Letter Sent and attach the correct file.
7. Click on Accept Changes and Save to save your changes.
