The Home Support Services Authorization History section pulls in a client’s Home Support per diem (LTC6 Per Diem) and Effective Date (Date of LTC6) from the client’s Financial Assessment. This information displays as read only.
To Print a Home Support Order
- Select a home support entry found within the Home Support Order Authorization History grid
NOTE: home support entries with a status of Active are the current home support orders that community health workers are following. A home support entry with a status of Inactive is a historic entry. - After selecting the home support entry that you want to view / print click on the Print Form button located in the upper right corner of the Home Support Order Authorization History grid. This will bring up the Report Options screen.
3. On the Report Options screen you will need to select which Report Version you want to view / print before you can run the report. After selecting the version, click Run Report.
NOTE:
- Both the Agency Version and Client Version will display all the tasks / care that community health workers are providing for the client. The main difference between the 2 reports is that the Agency Version contains financial information which is absent from the Client Version
- The Agency Version should never be given to a client or their family
Personal Assistance Guidelines (PAG) (also known as Transfer of Function [TOF] or Delegation of Task [DOT]) are required for delegable tasks for unlicensed care providers (UCP). Delegable tasks are tasks that do not fit into the standard role description of a UCP, usually performed by a licensed health care professional (eg. RN, OT, PT, RRT, etc), but may be delegated and performed by a UCP under certain circumstances (UCP training is provided, professional assessment and judgment is still in place, and professional supervision of the UCP) and if it is in the best interest for the client.
