Client Marked Dead Erroneously
The Death Application is found in Client Info. It has three functions:
1. It permits the documentation of the date and, if known, the time of a client’s actual death.
2. It permits the identification of the person that is confirming the death, and the date and time of that confirmation.
3. It permits an automatic notification to be sent to all staff members that are allocated to the client once the death is marked as confirmed.
IMPORTANT POINTS TO NOTE BEFORE USING THIS APPLICATION
1. Documentation of death in PARIS does not replace normal workflow/practices related to a client death. Before documenting the death in PARIS, consider how best to communicate the death of the client to other team members and staff who have been involved with the client.
NOTE: Once the client death is marked as confirmed in PARIS, an automatic notification will be sent to all staff members that are allocated to the client. This can be traumatic/triggering and ideally this should not be how staff first hear about the death.
2. Once a death is confirmed and saved it cannot be modified, changed, or deleted without contacting PARIS Support at 604-875-4334.
3. Once a death is confirmed, for any open referrals, the referral reason will change to client has died.
4. Be sure that the identity of the client is correct before marking them deceased.
5. If accidentally completed on the wrong client, call PARIS Support (604) 875-4334 immediately to reverse.
6. Documentation of death does NOT cancel a client care card.
7. The death application cannot be used to record the death of a non-client.
8. Completing this application for a client that has died does NOT:
- Close any open referrals
- End date addresses or telephone numbers
- Put an Alert in PARIS that client has died
NOTE: These actions will need to be completed separately if important to workflow/practices related to client death.
Document Client Death
1. First confirm client identity then verify your information source before confirming death in PARIS The following table lists ways in which staff can be informed of client death. The table is arranged so that more reliable information sources are at the top and less reliable sources are at the bottom. For all sources that fall below the red line it is important to verify and seek more information before confirming death in PARIS and document in a case note any decision making and actions taken regarding the information source.
2. NOTE: Though the data entry screen is titled Death Certification, completing it does not generate an official death certificate nor is it any way associated with an official death certificate. This is the data entry screen for death:
To document a clients death, enter the following fields:

- Date of Death: This is the date the client died; format is dd/mm/yyyy
- Time of Death: Enter time only if known; format is 24 hour clock hh.mm (e.g. 14.00)

Confirmed: Tick this box only when death is confirmed. When ticked, the ‘On’ and ‘At’ fields become mandatory. Follow site policy/workflow/guidelines for what is required for confirmation of death and who can complete these fields:
- On: Enter the date death is confirmed (not date of death); format is dd/mm/yyyy
- At: Enter the time death is confirmed (not time of death); format is 24 hr clock hh.mm (e.g. 14.00)
- Recorded By: Select the name of the person recording the death confirmation; defaults to your name - if this is incorrect select the correct person from the picklist
- Place of Death: If your team records place of death, select appropriate choice
- Notify Method: Enter method of death notification, select from picklist
- Client Status: Changes to 'Client has died' after save. This cannot be changed once saved. Contact PARIS Support immediately if entered in error.
NOTE: Once death has been confirmed, 'Client has died' appears in the Client Banner next to the name and PARIS ID of client.
Update Date of Death or Recorded by
1. Click on the Death Application and update the following fields:
- Date of Death: To change the date of death press F4 and select the correct date from the calendar,

Recorded by: If this is incorrect: Click
and select the correct person from the picklist
2. Click Accept Changes & press F12 to Save.
NOTE: This can only be done prior to ticking the Confirmed check-box
