PARIS TB Screening Assessment is used to document a client’s clinical assessment and TB skin test results. Based on the BCCDC TB Screening form, the version 1 assessment was built in 2018. As the screening form was updated over the years, the version 2 assessment was implemented in April 2023.
Once the assessment is completed, it can be printed to be sent to BCCDC TB Services, for the client’s own record, or as the requisition form for a chest x-ray. Additionally, a TB Skin Test Result report can be printed from the assessment.
The TB Screening Assessment has 4 tabs at the bottom of the form: Details, Screening Details, Test Results, and Document Upload
Details Tab
The Details tab contains the Header Details, Form Tools, Additional Demographics, and Physician (Or Primary Care Provider) sections.
The Header Details section captures the details of the assessment overall.
The Form Tools section is used to print the assessment. See the Reports section at the end of this User Help page for more details.
The Additional Demographics section is used to record additional client demographics.
The Physician (Or Primary Care Provider) section displays the current GP/NP information for the client. ‘’No Physician or primary care provider has been entered” is seen when no GP/NP is recorded for the client. To add or update a GP/NP, click on . Once it is updated in Central Index, click
twice at the right top corner to go back to the assessment form.
Screening Details Tab
The Screening Details tab is used to record all information collected when assessing the client prior to administering the TB skin test. This tab contains the Screening Details and Risk Factors & Symptoms sections.
The Screening Details section is used to record the reason for the tuberculosis screening and relevant history, including medication allergies. Once clinicians obtain medication allergies from the client, they are recorded in the free text field of the assessment. Clinicians must also review PARIS allergies in the Client Banner and update if appropriate.
The Risk Factors & Symptoms section is used to record the client’s risk factors, symptoms, and other relevant details. Any risk factor with an accompanying Specify field requires mandatory free text entry into the Specify field in order to save the assessment.
Test Results Tab
The Test Results tab contains the Test Results section, which is used to record the administration, test results, and follow-up recommendations of the TB skin test.
Clinicians can document a second TB skin test in this section of the TB Screening Assessment if clinically indicated for the client.
Based on the test results, a TB Skin Test Result letter can be generated in the Test Results tab. The generated letter will pull in the results of the most recent TST. To generate the letter, click the Generate TB skin test result report beside the More actions menu. See the Reports section at the end of this User Help page for more details.
Document Upload Tab
Clinicians have the ability to upload pdf documents related to the assessment from their Home Directory (H:). Only documents specific to the TB Screening appointment, such as a doctor’s referral letter, should be uploaded to the assessment.
- Maximum file size: 2 MB
- File names: the system does not allow duplicate file names. Please use the following naming convention: date, description of uploaded document, initials (e.g. 2021-01-12_drreferral_AW).
To Upload a Document:
- Insert a Row
- Click Attach File
- Highlight the file in your H:. Click Open.
- Document Date: enter
- Description: enter if required
- Click Accept Changes
To View a Document:
- Highlight the row in the grid
- Click View file button. File will open.
OR
Click View Details or Modify row. Click View file button within the entry screen. File will open.
To Enter In Error a Document
- Highlight the row in the grid
- Click Modify row
- Tick the Enter In Error box at the bottom of the Document upload – entry form
- Click Accept Changes
- Please note that the grid row will still be viewable in the Document Upload section; however, YES will appear under the Error column and you will no longer be able to view the file

TB Screening Assessment |
This report contains client demographics from Central Index and the clinical assessment and test results entered in the PARIS assessment. It mimics the BCCDC TB Screening form. This report can be printed and sent to BCCDC TB Services if required, and/or can be given to the client as their copy of the test results. |

X-Ray Requisition |
This report contains the same information as the TB Screening Assessment report, but it excludes the client’s risk factors and symptoms. This report acts as the client’s chest x-ray requisition if follow-up is required. |

Tuberculosis (TB) Skin Test Result |
This letter/report displays the client’s name, date of birth, test date, test result and interpretation from the most recent test entry per the assessment form. This report is generated upon request by clients, and would be given in the same context as when, previously, a copy of the TB Screening Form was provided to clients requesting documentation of their TST result. This client-friendly option for TST results does not contain any sensitive or confidential personal health history. The report also directs clients to the TB Skin Test health file for their reference and interpretation of TST result. |
