Temporary Address Workflow Steps
1) Change Address
2) Add Temporary Address Alert
3) Review, Modify or Remove Temporary Address Alert
1) Change Address
Whenever the client moves to a different address
2) Add Temporary Address Alert
When a temporary address has been added add an alert to the client’s banner.
Alert Code: TEMPAD or Alert Description: TEMPORARY ADDRESS-SEE ADDRESSES
Reason: enter the D/M/Y when the temporary address started, and the anticipated end date D/M/Y.
3) Review, Modify, or Remove Temporary Address Alert
When you see a temporary address alert:
1. Select View all alerts to Review the date the client was meant to return to the permanent address.
2. If the date has passed, review the client’s current address.
a. If client still residing at a temporary address – Modify Row to modify anticipated end date
b. If client has moved to a permanent address – Modify Row and Inactivate the alert to remove alert and ensure the updated address is entered in Central Index > Personal Details
| Points to Remember |
| Key Points | Rationale |
|
Whenever a client moves to a new physical location (permanent or temporary), update their address in PARIS’ Central Index >Personal Details
|
Ensures the Client Banner gets updated with the client’s physical address for clinician reference |
|
Addresses do not need to be labelled “Temporary”
|
While an address can be labelled “temporary” in the address grid, this address will not show up in the Client Banner |
| When saving a change, always choose “Replacement”, unless the previous address was entered in error and requires correction |
This ensures the client’s address history is maintained
|
| Keep Current box checked |
This ensures the address is sent to the Enterprise Master Patient Index, which is the central client registry which provides demographic updates to other health record systems
|
| Add a Temporary Address alert as per above | This ensures clinicians can review when the temporary address was meant to end |
| Review the Temporary Address alert | Ensures it is modified or removed as necessary |
Appendix: Definitions and Objectives
Defining Types of Addresses (Permanent or Temporary)
A client’s physical home address (where a client is currently staying) can be permanent or temporary.
Objectives:
1) Ensuring the Physical Address (Permanent or Temporary) is on the Client Banner
The PARIS Banner needs to reflect the clients physical home address to ensure clients receive visits from clinicians and home support at the address where they will be.
Permanent and Temporary Home Addresses can both be captured as the client's physical address in the same way in PARIS
2) Ensuring accurate data fed to other systems
Address data entered in PARIS is sent up to the Enterprise Master Patient Index (EMPI), which informs other electronic health record (EHR) systems.
