Enter Client's Name
Name: Enter person’s legal Last and First Name in appropriate fields
- Enter person’s Middle Name (if available)
Preferred Name: Enter preferred name if applicable
- If the person has known aliases or different ways of spelling their name(s), create an appropriate aliases in the Names section of Central Index
Update First or Last Name
This applies to wrong spelling and/or wrong names
1. Correct the name, press F12 (e.g. Client was entered as David Smath, should have been Smith)
2. On Save “The Name has Changed’ verification window appears
- If name was spelled incorrectly, select Correction
- Type for Replaced Name: Becomes mandatory
3. Press Enter & F12 to Save
- If you truly made a mistake & the name is the WRONG name, leave the radio button as Correction
Enter General Practice Provider (GP/NP)
1. Click the for GP/NP field, GP Search window will appear.
2. Enter the practitioners last name in the Last Name field, remembering to put an asterisk (*) after the providers last name to include specialists.
3. Click Search or press Ctrl + S on the keyboard.
4. Select the correct physician in the Search Results. Some physicians appear in the list more than once because they work in multiple clinics. Select the clinic where the client sees this physician.
Modify Family Provider/GP/NP
If the client now has a NEW Family Provider/GP/NP
1. In the Central Index Application, and the Personal Details form under the Details section edit the GP/NP field:

GP/NP: Click the
icon to launch the (GP) SEARCH window
2. Enter the practitioners last name in the Last Name field, remembering to put an asterisk (*) after the providers last name to include specialists.
3. Select the correct physician in the Search Results. Some physicians will appear in the list more than once because they work in multiple clinics. Select the clinic where the client sees this physician.
- Correction defaults into the ‘Change GP/Practice’ confirmation box
- MUST choose 'Replacement- previous kept in history' in the 'Change GP/Practice' Confirmation box to ensure history of prior provider
- Replacement: previous kept in history saves previous GP in history
- Correction: previous NOT kept in, use ONLY if current GP has never been this client's GP or current GP was entered in error
If the person has NEVER seen this GP/provider
1. In the Central Index Application, on the Personal Details form under the Details section edit the GP/NP field:

2. Enter the practitioners last name in the Last Name field, remembering to put an asterisk (*) after the providers last name to include specialists.
3. Select the correct physician in the Search Results. Some physicians will appear in the list more than once because they work in multiple clinics. Select the clinic where the client sees this physician.
- Correction defaults into the ‘Change GP/Practice’ confirmation box
- NOTE: When correction is selected, the previous GP/Provider will NOT be saved in History
4. Click Accept Changes.
No GP, Non-BC GP, Unknown GP, or Walk-In Clinic
If person has no GP, or you are unable to find the GP (Unknown GP) or attends a Walk-in Clinic
1. In the Central Index Application, and the Personal Details form under the Details section edit the GP/NP field

GP/NP: Click on the
to activate the (GP) SEARCH window
- Last Name: Type 'GP' and search (Ctrl + S)
2. Select appropriate response that appears in the Search Results section and click Select
3. Press F12 to Save
Enter an Address

Contact Details should contain the client’s phone numbers. Other numbers such as the brothers, sisters, friends, should be entered in Unregistered Contacts.
Enter Client's Contact Details
You can modify but not remove rows from Contact Details. To change a Home phone or any other number:
1. Click on the row of information that needs to be changed.
2. Click Modify Row and overwrite the existing information.
3. Press Enter then F12 to save changes.
Enter Email Address in PARIS
Enter the email in Contact Details:
1. Enter everything prior to the @ sign in the prefix.
2. Enter everything after the @ sign in the suffix.
3. Do not enter the @ sign itself.
4. Do not exceed 16 characters
Enter Client's Pronoun(s)
1. Click the icon to view the pronoun value(s) in the picklist.
2. Select the pronoun value(s) identified by the client.
3. Click Save or press F12 to Save.
4. Selected pronoun value(s) will appear in the client banner beside client name.
TIP: When client's identified pronoun(s) are not in the picklist,
1. Select OTHER, SEE TITLE/PRONOUNS and Save.
2. Go to Title/Pronouns to specify.
