Basic Principles
One site = one assessment; if new line inserted, End date current flow sheet and open a new one, even if Type remains the same (i.e. a second PICC line)
- Always remember to COPY the most recent grid row to make sure the updated info gets populated over; you are responsible for reviewing the copied information as well as indicating the care completed this visit
- NOTE: If copied info is incorrect (e.g. device type), you are able to adjust it
- The Access Device Specific section and the Due Dates will copy over (for reference and planning); each part of the form can be modified as needed
- The IV Flow sheet is for documenting IV care—medications that are administered/dispensed go in the MAD-R
IV Flowsheet -Entry
Access Device Specific
The section is the Header for the form; all values copy over as a Baseline Reference, but can be corrected as required.
1. The insertion measurement for PICC line refers to what it was at the time the line was placed; this info must be included in the referral
2. Cut/Trim Length is a specific measurement to be included for Proximal Valve PICC lines; this info must be included for this type of line
3. The # Lumens ticked equals the # of boxes for entry in the Line Patency section (Single Lumen = 1 entry, Double = 2, Triple = 3)

4. The Lumen(s) blank box is a free text space to ‘flag’ which lumen you are referring to when there is more than one; area copies over. Use color/lumen gage to note lumen; DO NOT use med or infusion method as descriptor.
5. The Subcutaneous Engineered Stabilization Device Present? tickbox indicates if a client has a SecurAcath.
6. The Method of Infusion selected will display the pump and/or gravity teaching fields in the Pump and Gravity Teaching section (otherwise the section will appear blank).
7. If an Extension Set or Connector is attached, tick the indicator box. This shows the Extension Set Change and Connector Change fields in the Line and Site Maintenance section (otherwise they don’t appear). You can then document when it was changed and when it is due to be changed:
Line and Site Maintenance
1. Total External Length at today's assessment will have a grey ‘read only’ number beside them; this is the measurement from the last entry. Your assessment from today’s visit will be entered to the left of the read-only values

Pump and Gravity Teaching
1. The fields that display here are dependent on the Method of Infusion selected in the Access Device Specific section. You can document when the teaching started, when the client and/or caregiver achieved competency, or if no teaching was provided. Details go in Progress Notes.
Pump Settings and Infusion Maintenance (Up to 3 Pumps)
1. Section will appear blank, click 'Add new delivery mode' button and select the delivery mode from the picklist.

2. Choosing each value from the picklist will trigger the relevant parameter settings to appear
3. Can only choose TPN once
4. Choose TREATMENT COMPLETED when the client was on a pump, but the pump has now been discontinued; doing this removes that particular pump information without removing all the grid entry information. This section will not copy over to the next grid entry on the subsequent visit.
5. Choose ENTERED IN ERROR when you have entered pump information in error (e.g. wrong client); doing this removes that particular pump information without removing all the grid entry information. This section will not copy over to the next grid entry on the subsequent visit.
Entered in Error: If you have completed the entire entry in error (e.g. wrong client), tick the tickbox.
