To View a Document
8 palliative documents can be uploaded into PARIS in the Registers/ Palliative node of the Clinical Summary.
- GP/NP Information and Contact form
- Hospice Admission Agreement form (NOTE: this is not the Hospice/LTC Consent form that is uploaded into the Consent Forms node of the Clinical Summary)
- Hospice Referral form
- MOST form
- No CPR form
- Notification of Expected Death form
- Short-Stay User Fee form
- BC Palliative Care Benefits Registration form
The BC Palliative Care Benefits Registration form can be uploaded in the Funding And Income Sources grid.
All the other documents can be uploaded in the Client Instructions for Health Care grid.
NOTE: The Funding And Income Sources grid can also be found Funding/ Eligibility node within the Central Index. The Client Instructions for Health Care grid can also be found within the Instructions – Health/ Finance node of the Clinical Summary.
Only PDFs can be uploaded to PARIS. Any PDFs uploaded into PARIS need to be select from a user’s Home Directory (H:). Files saved to a user’s desktop; documents folder; or V:, X:, Z: etc. drive will not be detected by PARIS.
- Maximum file size: 2 MB
- File names: the system does not allow duplicate file names. Please use the following naming convention: date, description of uploaded document, and client’s initials (e.g. March 9 2022 BCPBF EW).
To Upload a Document:
- Insert a row within the documents corresponding grid (mentioned above)
- Complete all mandatory fields found within the grid’s entry screen
- Select 1 of the 8 palliative document types from the Type or Detail picklist
NOTE: If the document type or detail that the user has selected has document upload enabled an Attach File button will appear - Select the Attach File button
- Select the corresponding PDF from the H: and click Open
- Accept changes and save.
To View a Document:
- Click on the row of that file in the grid
- Click View file button. File will open.
OR
Click View Details or Modify row. Click View file button within the entry screen. File will open.
