The Closed Care Plan Entries section is a Home Health (HH) application within PARIS that is used to review historical Interprofessional Care Plan (ICP) entries. When an active Need on the ICP is closed, it is moved to this section along with any associated SMART Goal(s) and Intervention(s) linked to that need.
The Closed Care Plan Entries grid displays:
- The closure dates for the Need, SMART Goal(s), and Intervention(s)
- The Intervention Outcome Reason recorded at the time of closure
This section allows users to easily reference, and review completed care plan components for historical and clinical context.
Users can also select a care plan entry from the Closed Care Plan Entries grid, at which point PARIS will display the full details of that entry in the Closed Care Plan Entries Details section. This interaction mirrors the behavior in the ICP, where selecting an entry from the Add & Review Needs, Goals & Interventions grid displays its full details in the Needs, Goals & Interventions section.
