Public Health has a variety of forms that contain the Document Upload section, which allow for related client files to be uploaded in PARIS. While the features of each individual Document Upload section may differ depending on the form, overall functionality is the same. For more information on what types of files you may upload to the client record, please contact your program lead.
Document Upload sections located within Assessments will remain editable until the Assessment form has been end dated. Please contact service desk if you need to make any modifications to the file after the assessment is end dated.

Staff can upload various documents (e.g. Assessment Data, Referral forms).
- File types: PDF files can be uploaded from the user's Home Directory (H:):
- Maximum file size: 2 MB
- File names: Please use the following naming convention: date, description of uploaded document, initials (E.g. 2021-01-12_RabiesCRF_AW).

To Upload a Document
1. Insert a Row
2. Click Attach File
3. Highlight the file in your H:. Click Open.
4. Document Date: Enter the appropriate date
5. Document Type: Select from available options
6. Description: Enter if required
7. Click Accept Changes
To View a Document
1. Highlight the row in the grid
2. Click View file button. File will open.
OR
Click View Details. Click View file button within the entry screen. File will open.
To Modify a Document or Mark Document as Entered in Error
1. Highlight the row in the grid. Click Modify row button.
2. Entry screen will open. Update details as required.
If document was entered in error, select the Entered in Error tickbox. Grid row will be flagged as Error and file will no longer be viewable.
