Home Support Responsible Clinician Report
There are several reports for the Home Support Orders application. These reports are used by clinicians, providers and external providers to help support the use of Home Support Orders Application. Please click the links to learn more about the individual reports.

HSO Printout for the Client
The Home Support Order Client Report is intended for Clients and displays an order in printed format. It also includes some client information displayed in the Provider Information application. Certain fields that are not relevant to the client are omitted from the report, such as tasks that have not been selected and Exceptions approvals.
This report can be found by pressing the Print Form button within Form Tools section.
HSO Printout for the Providers
The Home Support Order Agency Report is intended for Home Support Providers and displays an order in printed format. It also includes client information displayed in the Provider Information application. Certain fields not relevant to the provider are omitted from the report, such as tasks that have not been ordered and Exceptions approvals.
This report can be found by pressing the Print Form within Form Tools section.

Provider Notification Report
The Provider Notification Report is intended for Home Support Providers and shows important information about orders and other client information the Provider may need to act upon.
Information in this report includes:
- Orders that are awaiting review and confirm
- Previously submitted orders that have recently been cancelled
- Orders where the Authorized End date has been recently updated
- PAG/DOT letters in PARIS that are new or recently updated.
- Client information from the Provider Information application that has recently been updated
Providers have a schedule to run this report throughout the day. Additionally, standard response times for orders awaiting review and confirmation are established with each home support agency (internal and external). Refer to guidelines or local leadership for further information.
This report can be found by pressing the Reports button on the application toolbar.
HS Data Quality Report
The Home Support Data Quality Report is intended for Community Team staff (e.g. Program Assistants, Clinicians) and displays information the staff may need to act upon to maintain accurate data quality.
Information in this report includes:
- Clients with no active or submitted orders, which may indicate a need to discharge the client’s Home Support referral
- Clients with open orders to more than one agency (Provider). Clients typically should only be receiving service from one provider at a time, but there are instances when this is needed
- Draft orders that have not been submitted. This may indicate orders where the clinician has forgotten to complete and/or submit the order
- Submitted or Active orders where a Responsible Clinician has not been entered
This report can be found by pressing the Reports button on application toolbar.

Home Support Responsible Clinician Report
The Home Support Responsible Clinician Report is intended for Responsible Clinicians who oversee Home Support Service for clients. The report displays a summary of current Home Support Orders by a Responsible Clinician or for a team.
Information in this report includes:

A list of Submitted or Active orders, including whether the order has been confirmed or not

Clients with no active or submitted orders, which may indicate a need to discharge the client’s Home Support referral

Draft orders that have not been submitted. This may indicate orders where the clinician has forgotten to complete and/or submit the order
This report can also be run to identify orders where there Responsible Clinician has not been entered
This report can be found by pressing Reports button on application toolbar.
