You can enter allergies from the Client Banner, Clinical Summary, Central Index or a New Registration.
Add Allergies from Client Banner
1. To expand the Client Banner, click the purple arrow or click anywhere on that row.
The Client Banner expands.
2. When no allergies are recorded:
Click Add Allergies link in the Allergies column. The Allergies grid displays.
OR
When at least one allergy is recorded:
Click View all allergies link. The Allergies grid displays.
Entered allergies display in the Client Banner with the date the allergy was entered or modified. An Allergy indicator also displays in the Client Banner.
Enter Allergies
1. Click Insert a Row in the Allergies grid.
When you are a member of more than one team the Owner key lookup window displays.
- Select your correct team.
- The Allergies entry form containing 4 tabs displays on the Details tab.
NOTE: The Allergies form has four tabs identified at the bottom left of the form. You need to complete the Details tab and the Reactions tab when recording an allergy.
There are three steps to enter an allergy.
Step 1: Complete Allergy Details
Complete the Details tab for Drug Allergies - view video
1. Select Drug from the Allergies Type drop down.
The Drug lookup field displays.
2. Press F4 or click the lookup icon in the Drug (lookup) field.
The (Drug) Search pick list opens.
Best Practice: Search for the generic name first. (The list does contain a few trade names as well.)
3. Enter the Drug Search criteria by entering one of the following:
- Enter a Description (typically the generic name of the drug)
- Enter the first few letters of the description followed by a wildcard asterisk (*).
- Enter an asterisk (*) wildcard in the Description or Code field to view all drugs.
4. Click Search or press Ctrl + S.
The search results display in a list.
5. (Optional step) Use the Type & Find to further narrow your search results.
The search results decrease based on the characters typed.
6. Select the appropriate drug by double clicking on its row.
The drug displays in the Details tab. Adverse Reaction populates the Category field.
7. When you cannot find the drug in the pick list, you can type the drug in the Drug (free text) field.
BEST PRACTICE!
Sufficient effort should be taken to locate and select the correct allergen. The list is comprehensive but not necessarily complete. Make sure you have the correct spelling. Consult available resources such as CPS or Lexi comp on the VCH intranet to identify the trade or generic names.
When the allergen cannot be located type the correct spelling of the allergen in the free text field.
8. When required to change the Category:
- Press F4 or click the lookup icon
in the Category field.
The Category pick list displays.
OR
- Start to type the Category (e.g. Allergic Reaction, Anaphylactic Reaction).
A predictive list of options displays under the Category field. - Double click on the correct Category.
The revised category displays in the Category field.
9. Enter the Date of onset.
NOTE: This optional field can contain a date or other text such as "5 years ago".
10. Enter the Source of information using either the lookup icon , press F4 or using predictive text.
The Source of information selection list displays.
11. Double click on the correct Source of information (e.g. Client, physician).
12. (Optional step) Enter additional text in the Source notes field.
13. Continue steps to Verify Allergies for clinicians or Enter Allergy Reactions.
Complete Details tab for Food and Substance Allergies - view video
1. Select Food or Substance from the Allergies Type drop down.
The Allergy Type lookup field displays.
2. Press F4 or click the lookup icon in the Food (lookup) or Substance (lookup) field.
The Search pick list opens.
NOTE: The Common only tick box is checked. Maintain this selection to search common allergens only. You can deselect the Common Only check box and use the asterisk (*) to view the entire list of allergens.
3. Use the Type & Find feature to further narrow your search results.
The allergen list results decrease based on the characters typed.
OR
4. Enter the allergen search criteria by entering one of the following:
- Enter a Description

Step 2: Verify Allergy - view video
Each allergy needs to be verified by a clinician. When a clinician enters the allergy the Verification section can be completed at the same time the allergy is entered.
1. When necessary open the Allergies grid.
The Allergies grid displays.
2. Click Modify row for the allergy you need to Verify.
The Allergies form displays on the Details tab.
The Verification section comprises the bottom half of the Details tab.
3. Complete the Verification section as follows:
- Press Ctrl + T or enter a date in the Verification date field.
- Enter your user name in the Verified/ updated by field and press the tab key.
- Enter Verification notes in the text field when applicable to provide additional detail of how the allergy was verified. All fields populate with the added information.
4. Continue steps to Enter Allergies Reactions when not yet entered.
Step 3: Enter Allergy Reactions - view video
To add a reaction for a newly documented allergy or to add an additional reaction for an existing allergy:
1. With the Allergies form displayed, click the Reactions tab.
The Reactions grid displays.
2. Click Insert a row in the Reactions grid.
3. When known and available, enter the Date of reaction field with a date or text. (e.g. summer 2005)
4. Start to type the reaction type in the Reaction Type field.
A predictive list displays.
OR
Press F4 or click the lookup icon in the Reaction Type field.
The Reaction Type pick list opens.
5. Double click the Reaction Type.
The Reaction Type field displays the selected reaction type.
6. (Optional step) Enter text in the Reaction Details to provide additional details.
7. Best Practice to include. (Optional step) Start to type the Severity of reaction.
A predictive list displays.
OR
Press F4 or click the lookup icon in the Severity of reaction field.
The Severity of reaction pick list opens.
8. Double click the Severity of reaction.
The Severity of reaction field populates.
9. (Optional Step) Type user name or use the lookup icon to identify a person witnessing the reaction in the Witnessed by field.
10. When the source of information is different than the Source identified on the Details tab, use predictive text or the lookup icon in the Source of Information field to identify the alternate source.
The Source of Information field populates.
11. (Optional Step) Enter Source notes to provide additional details.
The details display in the Source notes field.
12. Click Accept Changes.
The Reaction form closes. The reaction appears in the Reactions grid on the Reactions tab.
13. Click Accept Changes.
The Allergies form closes.
14. When the allergy is not yet verified, a clinician needs to Verify Allergy.
