The purpose of the SAM Tool Screener (TOP Collaborative Initiative) is to identify, screen, develop and review treatment plan efficacy for new and current clients; who may meet the criteria for Treatment Resistant Psychosis (TRP). This new tool will support MHSU prescribers in determining if a client is eligible for a trial of clozapine medication for the treatment of Schizophrenia or Schizoaffective disorder.

The Sam Tool Screener can be used regionally by MHSU physicians and clinicians. This tool is available as its own Home Tile and will be available to all MHSU programs who have been apart of the Treatment Optimization Initiative which includes supervision and education.

There will only ever be one SAM screener active at one time, regardless of staff and or teams that work on the screener. This form can be updated collaboratively, and if multiple teams are completing the screener, the team will be captured in the Team field.

 

The S A M Tool has three sections: The S A M Tool Screener Summary, The S A M Tool Screener and the S A M Tool Screener Version History grid.

S A M Tool Screener Guidance 

There is a purple Guidance button located at the top left hand side of the Screener (see below screenshot). You can use this guidance button to locate guidelines and DST information. Ensure you review all appropriate guidelines and DST's prior to completing the screener.

S A M Tool Screener Summary

Describes the purpose of the tool and displays the status and outcome of the screener. Screening statuses will be updated as the Screener is completed to indicate up to date statuses and outcomes.
Screening Statuses = Screening in Progress, Intervention Required, or Screening Complete
Screening Outcome = To Be Determined, Adequate Response Achieved, Meets Criteria for TRP (Suitable for clozapine), Meets criteria for TRP (Not Suitable for clozapine) or Does not meet the criteria for TRP (Not suitable for clozapine).

S A M Tool Screener Entry Form

This is where you document the screening details. The form is meant to be completed over time and dependant on the answers chosen, the form will provide direction as to next steps.

All questions are greyed out initially and will activate as you move through the form from top to bottom providing answers to the questions.

As you move through each section of the form, you may save (Accept Changes) the form at any point. To continue/complete screening, you will need to click on purple Continue screening button. This will allow the current form in use to open, so you can change and or update your answers as applicable and you can repeat this step until all information is entered and the Screener is complete.

Each time the SAM Tool Form is saved with new/updated entries you will see these updated entries in the grid and in the SAM Tool Screener Version History grid.

Creating a new screener

1. First click on the Psychosis Screen & Manage Tool Home tile
2. To start screening, click on the S A M Tool Screener tile
3. Click Insert a row
4. Choose team you are creating the screener against and enter the following information:

        • Initial Screening Date: Todays date will auto populate. This date is the date the screener was first created.
        • Current Screening Date: Todays date will auto populate. This date reflects the current screening date, if this screener take more than 1 day to complete
        • Review Date: Enter any date to see a flag on the report for review
        • Team: Team selected will appear here
        • Screened By: Will auto populate logged in clinicians name
        • Transcribed By: Who entered the form on someone’s behalf NOTE: If you are filling this screener out on someone’s behalf (transcribing), enter the physicians name in the Screened By field and your name in the Transcribed by field

5. Fill out the rest of the form and based on the answers you select you will be enabled or disabled to continue screening. The screener will provide you guidance of any interventions you may need to provide with your client prior to continuing.

6. The Continue screening button gives you the ability to stop the screener at any time and “continue screening” at a later date.

7. Review all the below sections for information regarding each section of the screener.

 

Diagnosis
You will only be allowed to complete the screener if your client has Schizophrenia or Schizoaffective disorder. If your client has Psychosis NOS or no other relevant diagnosis you do not need to fill out this form at this time.

Clozapine Status
If your client is already on clozapine, you can still complete this screener up to this question. Once you tick yes, screening is complete and you can continue on the Clozapine Start tile to start charting the dosage and optimization of the client’s current clozapine trial.
Trialed Antipsychotic Medications
When entering the trial antipsychotic medications we only have the ability to enter two. Your client may have tried more than two but please enter the most recent antipsychotic medications here. You can choose to include a depot medication as one of the two medication, however you do not need to. You can list all other medications trialed, along with their outcomes in the medication outcome information field below.
Treatment History
Ticking either yes or no will allow you to move forward with screening. If no is ticked there is direction to consider adherence aids as a clinical queue, otherwise provide reasons for non-adherence in the reason for non-adherence box.
Psychoeducation
In order to complete screening you must provide formal psychoeducation to your client regarding the client’s mental health condition. NOTE: If you have not and no is ticked, you must first provide education then come back to the screener by clicking Continue screening. Now you can update your answer to yes. Upon clicking yes you will need to provide a date that the education was provided in order to move forward. This date is a mandatory field for reporting purposes.
Depot
If you have not offered a depot, there is direction provided for you to consider to offer/start a depot, if appropriate, and return to screener to indicate response to depot if depot has been started.

        • You do not need to offer your client a depot in order to determine if your client meets the criteria for treatment resistant psychosis
        • If you are not offering your client a depot or if you have offered but the client has not accepted, provide a reason in the additional information section below.

Treatment Resistant Psychosis Outcome Summary
Criteria for Treatment Resistant Psychosis = Clients who have either A) Tried two anti-psychotic medications and have been adherent to treatment without an adequate response or B) Tried two antipsychotic medications, have been non-adherent but have been offered or trialed a depot medication (if appropriate) without an adequate response.

        • If you select no to either of the questions in this section you will need to provide a reason as to why they did not meet criteria for TRP or why they are not suitable for clozapine in the Additional Information box
        • If your client does meet the criteria and is suitable for clozapine, you can continue to the Clozapine Start tile

SAM Tool Screener Version History

This section shows all the version histories of the particular form that is highlighted in the S A M Tool Screener grid. This screener could take weeks to months to complete, depending on what interventions are needed for the client. That is why this form has the unique “Continue screening” button that allows you to start screening, save and continue screening at a later date if needed. The version history shows all the times that “Continue screening” was clicked and shows what was entered at each time. This view will show you the clinician who screener the client along with the team.
